No, you cannot currently add a new Assessment template for Incident reporting; however, you can edit the existing templates to suit the information you would like to gather.

• On the left-hand navigation bar, click Settings > Incident > Assessment Creator, and refer to the 'Actions' tab beside the Assessment template you would like to edit. 
• Click Edit.
