Register Text appears in the Incident Reporting, while Survey Text is displayed within the Incident Assessment.

In many cases, the Register and Survey dropdown option text will be the same; however, in some cases, you can have technical terms that end users may not understand.  With the Survey Text, you can provide the end user with a layman's version of the technical term, so it is easier for them to understand; but, within your Incident Reporting, the actual technical term will appear.
