In order to attach a survey as a Learning Experience, you first need to navigate to Policy > Survey Creation. 

• Next, click on Create as Attached Survey:

• Once this has been published, you should then navigate to Phish > Phish Creation > Learning Experience > Content Type:

• Finally, select the survey you require from the drop-down options provided.
